Requirements for notarization

Prepare these before going to the office so the notarization can be completed without delay.

1. Valid government ID

Bring an unexpired government-issued ID with photo and signature, such as Passport, Driver’s License, UMID, SSS, PRC, GSIS, Postal ID, Voter’s ID, Senior Citizen ID, or other accepted ID.

2. Copy of document and ID

Bring the original document plus photocopies of the document and your ID. The notary keeps copies for the notarial file and register.

3. Personal appearance

All parties who sign the document must personally appear before the notary public. Documents are signed or acknowledged in the notary’s presence.
